Retail Applications and Forms
Thank you for your interest in Paradise. If you are interested in applying for an account with us please read the New Account procedures:
- Please completely fill out the New Account Application. Any incomplete information will only delay account set up.
- Please sign and return a copy of the New Account Application to Paradise via fax, email, or regular mail.

- Please return the following along with your application:
- Retail stores and on-line retailers inside California will need a reseller number as well as a California Resale Certificate form completely filled out
- Retailers and on-line retailers outside California need to provide a Federal Tax ID and a copy of your State Sales/Use Tax Permit/Certificate or a City/Town/County Business License.
- Practitioners/Physicians/Clinics need to provide a valid Practitioner or Physician license.
- Account will be approved and set up once all forms have been received and the first order is placed.
